Amherst Veterinary Hospital offers our patient form(s) online so you can complete it in the convenience of your own home or office.

• In order to streamline your pet's visit for the following procedures, please fill out the appropriate forms prior to their visit.  We will be happy to review the forms with you and answer any specific questions.
  

New Client Information form

This lets us record our pet and owners into our management software so that we can effectively and efficiently provide the service you deserve!

Canine Anesthetic/Surgical Consent form

This form is required for any dog that will be undergoing any anesthetic/surgical procedure.

Feline Anesthetic/Surgical Consent form

This form is required for any cat undergoing any anesthetic/surgical procedure.

Dental Consent form

This form is required for any pet undergoing a dental procedure.

Boarding Consent form

All pets that will be boarded will need to have the following form completed.

Canine Behavior Consultation Questionnaire

All dogs scheduled for a behavior consultation must complete and return this form 3 days before the appointment.
