Before we can begin the process of addressing your pet's behavioral problem, you will need to complete a behavior consultation questionnaire. Forms are available at the office or online (www.AmherstVetS.com) under the "New Patient Center" section. We need to receive the completed questionnaire at least 4 days prior to your appointment. You can bring the form in, fax it (434-929-2416) or send via email (BLuzaich@AmhersrVetS.com).
While we are able to help you with most behavioral problems, there remain some issues where referral to a board certified veterinary behavioral specialist may be required.
